As part of your medical home, your physician and our behaviour health consultants are committed to supporting our patients through many of life’s transitions, by helping you develop habits to cope with stress, worry, or emotional concerns that may interfere with your overall health and wellbeing. 

Who Are We

Behavioural Health Consultants (BHCs) can help you when your habits, thoughts, behaviours, emotions (or any combination of these) impact your daily life or overall health. They’ll meet with you and help create a plan that works for you to reach your goals. Psychiatric Consultants are here to provide a one-time consultation. They will work with your primary care provider to offer recommendations aimed at supporting your mental health and overall well- being.

What We Do

Our Mental Health and Wellness program is here to support your overall health journey, working directly with you on a short-term non-urgent basis to provide mental health support that helps improve your well-being and health outcomes.

Common Reasons for Referral by Your Primary Care Provider

By working closely with your primary care provider, behavioural health consultants teach you coping strategies, goal setting skills and how to make action plans to better manage concerns, such as:

· sleep issues

· chronic pain and illness

· headaches

· depression

· anxiety

· weight concerns

· relationship and parenting issues

· substance use and addictions

· anger, grief, and stress

Who is Eligible

To access one-on-one mental health services, your primary care provider must be affiliated with the EWPCN, and a referral is required. Once the referral is received, our team will contact you.
