Primary Care Nurses

The EWPCN has nurses providing services in over 50 clinics to meet a patient’s needs and health care goals. They are key members of the patient’s medical home as they work with the family physician to provide comprehensive primary care.

Primary Care Nurses

Primary Care Nurses are able to provide support to patients for a variety of health care needs such as: 

  • Help managing chronic diseases like diabetes, high blood pressure, COPD, asthma, arthritis and more
    • monitoring and follow up
    • working with other health care professionals through team-based care
    • referrals to other resources
  • Women’s wellness
    • pap tests
    • information about birth control options
    • sexually transmitted infection screening and education
  • Pregnancy and postpartum care
    • breastfeeding support
    • well-baby care and check ups
  • Education and goal-setting about healthy lifestyle choices
    • healthy eating
    • physical activity and exercise
    • healthy aging
    • smoking cessation
    • weight management
  • Injections, wound care, suture removal, ear flushing and wart treatments
  • Mental health support and referrals
  • Advanced aging and memory tests
  • Assistance with annual screenings
  • Medication review
  • Release from hospital follow up
