At the Edmonton West Primary Care Network, family physicians work with a team of dedicated health professionals, made up of nurses, pharmacists, behavioural health consultants, dieticians, exercise specialists, social workers, nurse practitioners and more, all to provide patients with the care they need, through every stage of life.

The EWPCN is excited to announce the launch of the new Community Health Navigator (CHN) Program, which is designed to enhance patient-centered care by addressing non-medical barriers to health.
This Program acts as a trusted partner in helping patients access the care, resources, and support they need for better health outcomes. CHNs play a vital role in promoting longer-term well-being by empowering patients to navigate the health journey.

The EWPCN has partnered with Kickstand to support primary care services in Edmonton for youth aged 11 – 25 years old. The hub, located at the West Edmonton Mall, provides free mental health and substance use counselling, peer support, Indigenous wellness services, employment support and primary health care. A Nurse Practitioner will be onsite for walk-ins one day a week starting in April.

Congratulations to the EWPCN Medical Clinic!
The clinic celebrated its one year anniversary on January 15, 2025.
The EWPCN’s Nurse Practitioners (NP) have attached 967 patients who did not have a primary care provider. They provide comprehensive primary care services to patients of all ages and can provide assessment, diagnosis and treatment of medical conditions. The NPs also have access to the PCN’s multi-disciplinary team, such as Exercise Specialists, Dietitians, Social Workers, mental health specialists and other primary care providers.
